Registration Overview

USA Hockey Number for 2026-27 season (the player’s new USAH Registration Confirmation number for 2025-26 season will need to be entered)
USAH Consent to Treat (a scan or photo of the completed and signed document will need to be uploaded)
You will be asked to acknowledge review and consent with the waiver forms. Links are included during registration and are also provided here (USAH Player Code of Conduct, USAH Concussion Waiver, USAH Waiver of Liability, and WIHC Parent/Guardian Code of Conduct) in case you would like to review before beginning the registration process.

Pay in full upon registration -OR- Payment Schedule (payments are accepted via ACH electronic transfer from your bank, credit card, PayPal, or check)

Due at Registration: $200
Due by Sept 1st: third of balance
Due by Oct 15: half of remaining
Due by Dec 1: remaining balance
